The responsibilities of the position include: Training and support : Develop ing and conduct ing education with staff in NDIS-aligned care. Assess ing supervisors' competency in medication management. Care coordination and monitoring : Conduct ing assessments and monitor ing care plans. Track ing medical incidents and medication errors. Clinical practice leadership : Lead ing clinical practice’s and guid ing support workers and service managers. Promot ing health and support preventive care initiatives. Health care planning : working to personalised care plans, including complex needs. Ensur ing external support plans comply with NDIS standards. Collaboration with teams : Work ing closely with teams to ensure high-quality care and compliance with NDIS standards.
