Role Description This is a remote contract role for a Telehealth General Practitioner with Horizon Health Care Partners Australia. As a GP/ NP you will provide virtual consultations to patients, delivering holistic care solutions including chronic disease management, mental health support, Weight Loss, Longevity / complete Wellness, preventive therapies and Holistic Care. Responsibilities include assessing patient needs, recommending treatments, collaborating with a multidisciplinary team, and maintaining accurate and confidential medical records. You will also empower patients to take proactive steps in their health and wellness journey. Qualifications Current unrestricted registration as a Practitioner with AHPRA. Experience in Medicinal Cannabis, Weight loss, chronic disease management, mental health care, or integrative therapies and Wellness program/ Longevity / Peptides. Strong telehealth consultation skills, including patient communication and clinical assessment via digital platforms. Proficiency in medical documentation, record-keeping, and maintaining patient confidentiality. Compassionate and patient-centred approach to care, with strong interpersonal and communication skills. Ability to work independently in a remote environment with reliable internet connectivity and computer skills. Commitment to ongoing professional development and staying updated on evidence-based practices. Prior experience or training in telehealth, preventive health, or lifestyle medicine is an advantage. Flexibility to work effectively in a virtual, multidisciplinary team environment.
