Xavier Children’s Support Network Brisbane, QLD | Community-Based Role | Full-Time or Part-Time Opportunity Xavier Children’s Support Network is seeking a dedicated and compassionate Accredited Exercise Physiologist to join our growing multidisciplinary team supporting children living with disability, developmental delay, complex needs, and older adults across the Eight Mile Plains, Ipswich and Logan areas. About the Role As the Xavier Exercise Physiologist, you will provide high-quality exercise physiology and rehabilitation services designed to improve physical function, independence, participation, and wellbeing. You will work collaboratively with clients, families, carers, support networks, and multidisciplinary professionals to deliver tailored, goal-focused programs that support people to thrive across all stages of life. Key Responsibilities Deliver evidence-based exercise physiology and rehabilitation interventions Conduct functional assessments including mobility, balance, endurance, falls risk, and daily living skills Develop and implement individualised, goal-focused therapy programs Deliver services across home, school, community, clinic, and aged care environments Monitor client progress and adapt interventions to support outcomes and changing needs Utilise adaptive equipment and assistive technology to support safe participation Maintain accurate clinical notes, reports, and funding documentation Complete NDIS and aged care assessments, reports, and compliance requirements Manage caseloads, scheduling, and service delivery within funding guidelines Collaborate with families, carers, support workers, and multidisciplinary teams Provide education and capacity-building support to carers and support staff Participate in supervision, professional development, quality improvement, and clinical governance activities Promote safe mobility, participation, independence, and wellbeing for clients About You You are a compassionate clinician who values inclusion, collaboration, and evidence-based practice. You thrive in community settings, enjoy working with diverse client groups, and are confident managing a varied caseload independently. Essential Requirements Tertiary qualification in Exercise Physiology and current ESSA accreditation as an Accredited Exercise Physiologist (AEP) Experience in disability, paediatrics, community health, and/or aged care settings Knowledge of neurorehabilitation, falls prevention, autism-informed practice, sensory approaches, and reablement principles Strong clinical reasoning, communication, and stakeholder engagement skills Experience supporting people with complex behaviours, cognitive impairment, and mental health co-morbidities Understanding of aged care frameworks including Support at Home and CHSP Current Queensland Driver Licence and access to a reliable vehicle Current Blue Card, NDIS Worker Screening Clearance, National Police Check, and Medicare Provider Number (or willingness to obtain) Australian working rights, citizenship, or permanent residency Why Join Xavier?
