Clinical Care Partner (Registered Nurse) – Community Aged Care Part-Time | East Perth & Surrounding Communities Are you a compassionate and resilient Registered Nurse looking to make a genuine difference in the lives of vulnerable people within the community? We are seeking a Clinical Care Partner to join our Support at Home team, delivering high-quality clinical oversight and person-centred care to older Australians living independently in the community. This is a newly created role that combines clinical expertise with relationship-based case management, supporting some of our most vulnerable clients, many of whom have experienced homelessness, mental health challenges, alcohol and other drug (AOD) issues, and complex health needs. About the Role As the Clinical Care Partner, you will work closely with the Support at Home Manager and Care Partners to ensure clients receive appropriate, coordinated, and clinically informed support. You will manage a small caseload of approximately 4–6 high-needs clients while providing clinical oversight and guidance across the broader Support at Home program. This role is offered on a part-time basis , with flexibility to work either: Three full days per week; or Reduced hours across five days. All client visits are located within approximately 30–40 minutes of our East Perth office. Key Responsibilities Manage a small caseload of clients with complex physical, psychosocial and environmental needs. Conduct clinical assessments and contribute to care planning. Review and oversee care plans across the Support at Home program. Monitor client wellbeing and identify emerging risks. Work collaboratively with Care Partners and external service providers. Support clients to remain safely and independently within their homes and communities. Maintain accurate clinical documentation and reporting. Travel independently to client homes and community locations. About You To be successful in this role, you will be a Registered Nurse who is comfortable working autonomously in community settings and confident supporting people with complex and often challenging life circumstances. You will possess: Current AHPRA registration as a Registered Nurse. Strong clinical assessment and care planning skills. Excellent communication and relationship-building abilities. The ability to work independently and make sound clinical decisions. Resilience, adaptability and a non-judgemental approach. Current driver's licence and reliable transport. Highly Regarded Experience in community aged care or Support at Home programs. Experience supporting vulnerable populations, including people experiencing homelessness, mental health challenges, substance use issues, or social disadvantage.
