About CRAICCHS: Cherbourg Regional Aboriginal and Islander Community Controlled Health Service (CRAICCHS) is a Aboriginal and Torres Strait Islander community-controlled organisation. CRAICCHS objective is to provide access to holistic and client-centred quality primary health care and social and emotional wellbeing services to the Aboriginal and Torres Strait Islander communities in the South Burnett regions. Key Duties: The duties will be diverse, and although not limited to, will include: Engage with adolescents to provide support through case planning and involvement in developing family plans. Develop case plans with families that address individual family-related risk factors that contribute to youth entering or re-entering the youth justice system. Develop and facilitate individual and group programmes to encourage improved behaviour, lifestyle changes and re-engagement with kin networks, schools and community. Participate in IYFW Reference Group meetings managed by the Aboriginal & Torres Strait Islander Commissioning Team. Advocate, negotiate and support families to make culturally based, family-driven decisions in the best interest of children through Aboriginal and Torres Strait Islander Family Led Decision Making processes. Collect and input client data and case plans into the Advice, Referrals and Case Management (ARC) online reporting system. Monitor, review and evaluate case plans with clients within established timelines to develop exit strategies and transition from support. Participate in Interagency Case Management meetings. Actively plan and deliver community development-based activities that promote safety and wellbeing of Aboriginal and Torres Strait Islander families and youth. Undertake other duties as required, that are within the scope of your skills, competence and training. Key Requirements: The successful applicant will be able to demonstrate: Although not an essential requirement, preference will be shown to applicants who identify as an Aboriginal or Torres Strait Islander person under section 25 of the Anti-Discrimination Act 1991. Relevant qualifications such as Cert III in Youth Work or Cert III in Aboriginal & Torres Strait Islander Primary Health or equivalent relevant experience. Competent computer skills for the use of MS Office, electronic client records, information management systems and quality management systems. Proven communication skills to effectively engage sensitively with Aboriginal and Torres Strait Islander people, those with disability and diverse characteristics and life experiences. An understanding of issues affecting Aboriginal & Torres Strait Islander societies and cultures The ability to maintain professional boundaries and comply with strict confidentiality policies. The willingness to learn and complete ongoing professional development and training. Knowledge / experience of juvenile justice, child and youth development, safety planning, youth mental health and working with hard-to-engage Indigenous youth will be a benefit.
