About CRAICCHS: Cherbourg Regional Aboriginal and Islander Community Controlled Health Service (CRAICCHS) is a Aboriginal and Torres Strait Islander community-controlled organisation. CRAICCHS objective is to provide access to holistic and client-centred quality primary health care and social and emotional wellbeing services to the Aboriginal and Torres Strait Islander communities in the South Burnett regions. Key Duties: The duties will be diverse, and although not limited to, will include: Working within the clinical scope of practice and care coordination including triage, screening and assessments, examination, treatment, case management, referrals, handovers, recalls, and case conferences, along with contributing to the development of models of holistic health incorporating continuity of care. Undertaking a wide range of clinical duties including immunisations, pathology sample collection, wound management, medication administration, patient screenings and assessments, within well-established clinical pathways, protocols and guidelines. Initiating GP Chronic Condition Management Plans and reviews, performing history, examination and relevant monitoring procedures, and collating required information for GP completion. Providing support, direction, orientation and education to newer or less experienced staff including EN's, Aboriginal Health Workers, student placements and other healthcare workers. Contributing to the achievement of National Key Performance Indicators (nKPI) for Aboriginal and Torres Strait Islander primary health care. Following guidelines necessary to optimise Medicare services activity, claims and compliance. Completing all reporting activities and documentation within the scope of role, ensuring they meet legal, professional and organisational standards and all patient-related records and documents are stored and managed in accordance with privacy policy. Implementing and monitoring systems and processes in the clinical environment to ensure they meet relevant standards, legislative requirements and internal procedures in infection control, cold chain monitoring, medical storage, clinical equipment, records management, work health and safety and accreditation processes. Assisting in the management of action research projects and participating in quality assurance programs and policy development within the clinic setting. Completing all required education and training programmes. Performance of other duties as required that consistent with the scope of practice and qualifications held. Upholding privacy and confidentiality procedures and regulations. Key Requirements: The successful applicant will be able to demonstrate: Nurse registration with current unconditional AHPRA registration and professional indemnity insurance. Demonstrated clinic-based knowledge and experience as a Registered Nurse within a primary health care setting, delivering responsive, evidence-based comprehensive care. An understanding of and commitment to the principles of Aboriginal community control, cultural identity and culturally safe care. The ability to apply critical thinking and analyse nursing practice, providing safe, appropriate and responsive quality nursing practice and manage competing demands. High level computer skills with Microsoft Office applications, electronic client record/information management systems and quality management systems. High level communication skills with the ability to engage and communicate sensitively with Aboriginal and/or Torres Strait Islander people, people with disability and those with diverse characteristics and life experiences. The ability to maintain professional boundaries and to work within, and maintain, strict confidentiality guidelines and policies. A commitment to ongoing professional development and the completion of required mandatory training. Flexibility to work or travel within the region to meet the specific needs of the role.
