About the Role: The Occupational Therapist will work amongst a multidisciplinary team and have sound knowledge of how the NDIS funding works and how to break down a plan. This role will entail outreach service delivery from Community-based settings. Daily tasks will include; Provide high-quality occupational therapy assessment and intervention for clients. Develop intervention plans, including short and long-term goals, in line with the client’s NDIS goals. Good communication, by ensuring integrated service delivery and follow-ups, coordinating and communicating as required. Incorporate a holistic approach to health care according to community and organisation priorities. Ensure all information is recorded accurately and in a timely manner in the electronic health record system. Work closely with support coordinators and referring clinics to track the client's progress. Ability to work autonomously Essential Criteria: AHPRA fully registered Occupational Therapist National Police Check, Blue Card (WWC) NDIS worker Screening Check (Yellow Card), (or relevant exemption) Demonstrated right to work in Australia Completion on NDIS Worker Orientation Certificate of completion must be provided.
