Clinical Care Coordinator (APS6) - Mental Health | Veteran Support The Opportunity Join a highly respected national organisation delivering critical mental health support services to veterans and their families. As a Clinical Care Coordinator (APS6), you'll work at the frontline of care-supporting individuals with complex mental health and psychosocial needs and helping them engage meaningfully with the services they need. This is a hands-on, client-focused role combining clinical intervention, case management, and stakeholder engagement within a supportive multidisciplinary environment. What You'll Be Doing Deliver short-term, goal-focused clinical case management to clients with complex needs Provide brief interventions, risk assessments, care planning and referrals Support clients to overcome barriers to accessing mental health care Work collaboratively with internal teams and external service providers Build strong relationships across health, community and government stakeholders Maintain accurate client records using case management systems Contribute to community engagement, programs and service development initiatives About You You're a skilled and empathetic clinician who thrives in a fast-paced, purpose-driven environment.
