You will be working within a collaborative, multidisciplinary team that includes Palliative Care, Nurse Led Clinic, Wound Management, Child and Family Health Nursing, General Practitioners, and a wide range of outreach services. These outreach services include Continence, Diabetes Education, Dietetics, Aged Care, Paediatric Speech Therapy, Paediatric Physiotherapy, Paediatric Occupational Therapy, Child and Family Counselling, Mental Health, and Women?s Health. This integrated approach ensures holistic, patient-centred care that meets the diverse needs of the community. What you'll be doing As a Community Registered Nurse, you will provide a high standard of person-centred care to clients requiring wound management and palliative care support. Care will be delivered across both clinic-based settings and in patients? homes, ensuring flexible and responsive service delivery. You will apply evidence-based practice and critical thinking to assess, plan, implement and evaluate individualised care plans. Working closely with the multidisciplinary team, you will contribute to improving patient outcomes while supporting patients and families through complex and often sensitive care needs. This position is supported by the Nurse Unit Manager, with additional specialty support provided by the Wound Management Specialist and Clinical Nurse Educator.
