We are currently seeking a compassionate and dedicated Community Worker to join our team. Position Overview As a Community Worker, you will support clients by assessing their individual needs and coordinating appropriate community and welfare services. You will work closely with clients, families, healthcare professionals, and community organisations to improve quality of life and ensure access to suitable support programs and services. Key Responsibilities Conduct interviews and assessments to determine clients’ physical, emotional, social, and support needs Assess the nature and extent of difficulties experienced by individuals and families within the community Develop, implement, and review individual support and care plans in consultation with clients and their families Monitor client progress and evaluate the effectiveness of support services and programs Identify additional support requirements and refer clients to appropriate agencies and service providers Liaise with healthcare professionals, community organisations, private businesses, and government agencies to coordinate support services Assist clients in accessing health, welfare, housing, employment, and community participation services Maintain accurate client records, case notes, and progress reports in accordance with organisational policies Promote social inclusion, independence, and community participation for clients Assess community needs and available resources relating to health, welfare, housing, and other support facilities Skills and Qualifications AQF Diploma or higher qualification in Community Services, Welfare, Disability, Aged Care, or related field Strong passion for supporting vulnerable individuals and improving community wellbeing Excellent interpersonal and communication skills Ability to provide culturally sensitive and person-centred support services Strong organisational, problem-solving, and case management skills High level of integrity, professionalism, and reliability Ability to work independently and collaboratively within a team environment Fluent English communication skills; bilingual abilities will be highly regarded Preferred Requirements Previous experience in community services, disability support, aged care, or welfare support Current National Police Check Current First Aid Certificate Valid Driver Licence preferred Why Join Us?
