Where clinical needs arise, they partner with qualified clinicians for complex care planning.As a Care Partner, you will:Develop individualised and culturally appropriate care plans aligned with assessments, goals, preferences, risks and AT HM needs; ensure care plans identify wellness and reablement approaches to meeting participant goalsCollaborate with relevant team members to support the coordination of service under the End of Life and AT HM Pathway (within scope), ensuring alignment with participant goals and safe integration of supportsProvide essential direct care services to participants as required, in response to individual care needs and service demand (such as personal care, medication support and transport).Deliver monthly care management interactions to all participants Who we're looking forYou're a natural people leader with a heart for community care and a talent for getting the best with your team. Our ideal candidateCertificate 3 in Home and Community or Individual Support One years' experience in home care and planning, co-ordinating and implementing care services in the home care environmentHLTHPS006 - Assist clients with medicationHLTAAP001 - Recognise healthy body systemsCHCCCS011 - Meet personal support needsCurrently holds, or willing to obtain a First Aid and CPR qualificationCurrently holds, or willing to obtain a current Police CheckA current driver's license and willingness to use your own comprehensively insured vehicle for work purposes.
