The important parts: Pay: $67.24–$76.36 per hour Super | ⏳ Duration: 24-month contract Hours: Monday–Friday (37.5 hours per week) Location: Surry Hills, NSW Applications close: Friday 5th June 2026 What you will be required to do: As a Clinical Care Coordinator, you will play a critical role in supporting veterans and their families to access and engage with mental health services, addressing barriers and improving overall wellbeing. You will: Provide short-term, goal-focused clinical case management Support clients with complex psychosocial needs and barriers to care Conduct risk assessments, care planning and brief interventions Coordinate referrals and connect clients with appropriate internal and external services Build strong relationships with community providers, DVA and support services Work collaboratively within a multidisciplinary clinical team About You You bring strong interpersonal skills, empathy, and the ability to engage with diverse stakeholders. You thrive in community-facing roles and are passionate about making a genuine difference. Experience in community services, case management, veteran support, or government programs will be highly regarded. Australian citizens can only apply.
