About the Position: The Community Worker plays a vital role at Star Community Care Pty Ltd by facilitating community development initiatives and providing direct, individualised support to help participants achieve their goals. This position involves assessing needs, implementing personalised programs, and assisting with daily living activities, community access, and social inclusion. As a key member of our team, you will contribute to empowering participants to live safely and independently while building stronger community connections in line with our holistic, participant-focused approach. Key Responsibilities: Assess participants' needs and co-develop tailored support plans that align with their goals for daily living, health, and community participation. Provide practical assistance with household tasks, personal care, meal preparation, and transport to support independence at home and in the community. Facilitate access to community activities, social events, and local resources to promote inclusion and wellbeing. Monitor participant progress, maintain accurate records, and report outcomes to ensure continuous improvement in supports. Liaise with families, carers, other service providers, and agencies to coordinate holistic care and referrals. Advocate for participants' rights and contribute to community development initiatives that address local welfare needs. Requirements : Australian citizens and permanent residents are encouraged to apply. Diploma or higher qualification in Community Services, Human Services, or a related field. 1–2 years of relevant experience in community support, disability services, or aged care. Strong interpersonal and communication skills, empathy, cultural sensitivity, and the ability to build trusting relationships with participants and families. Reliable transport, flexibility with hours, and a current Working with Children Check (or willingness to obtain).
