About the Role We are currently looking for an experienced Families Together Practitioner to join our team. Key duties will include: Engaging families through strengths based and culturally safe relationship building Delivering intensive family support through home visits and community outreach Undertaking holistic strengths, needs, and risk assessments Developing and reviewing family led case plans and safety plans Providing parenting support through coaching, modelling, psychoeducation, and advocacy Monitoring child safety and responding to changes in risk, including mandatory reporting Working collaboratively with Therapeutic Clinicians and multidisciplinary stakeholders Coordinating referrals and liaising with DCJ, schools, health services, and community agencies Maintaining accurate case notes, assessments, reporting, and outcome data This role is intended to be a fixed term contract for 12 months with potential for ongoing employment thereafter.
