With a booming, culturally rich and diverse community, and less than an hourâs drive from the CBD (still within the bounds of Myki and ride-sharing), Melbourneâs North is fast becoming the place to be.   Northern Health are seeking applications from motivated and suitably experienced Senior Social Worker Grade 3 to join the Broadmeadows Adult Inpatient Unit.  Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourneâs rapidly growing outer north.  Services are provided through our six main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.   About the Role:  The Grade 3 Social Worker will provide comprehensive psychosocial assessment, intervention, and discharge planning for patients within the Northern Health Broadmeadows Adult Inpatient Unit.  The role has a strong focus on effective discharge planning and the facilitation of engagement with appropriate community supports to ensure continuity of care.   This position is responsible for: This role involves monitoring and facilitating of the progress of people through the course of their inpatient stay from admission to discharge, with a specific focus on maintaining and establishing robust supports in the community which will enhance recovery and help prevent relapse. The incumbent will need to be self-motivated, able to work independently and assertively within and across service systems to strengthen the discharge planning process. The incumbent will need excellent interpersonal, therapeutic and liaison skills to prepare a consumer for discharge and to work collaboratively with the personâs System of Supports (SOS), including their family, carers and other service providers such as accommodation, education, financial and legal, to enhance the supportive scaffolding for each person.
