You will work closely with clients, families, care workers, allied health professionals and external providers to deliver safe, effective and culturally appropriate services while supporting quality outcomes across the community team. Key Responsibilities Conduct client assessments and develop person-centred care plans Coordinate and monitor community support services Provide leadership, guidance and support to Community Care Workers and field staff Support clients using a wellbeing, reablement and restorative care approach Manage client budgets and monitor service delivery outcomes Identify risks and implement appropriate support strategies Maintain accurate documentation and compliance with aged care standards Liaise with internal teams, families and external stakeholders Contribute to continuous improvement and quality initiatives Participate in reporting, team meetings and organisational change initiatives About You You are a compassionate and organised professional with strong leadership, case management and communication skills. You thrive in a fast-paced community environment and are passionate about supporting older people to achieve positive outcomes.
