About The Opportunity The Nurse Practitioner (NP) / Transitional Nurse Practitioner (TNP) is an advanced practice nursing role responsible for delivering high-quality, evidence-informed clinical care to consumers accessing the Drug and Alcohol (D&A) Service. The role provides comprehensive assessment, diagnosis, treatment, care coordination, health education, and therapeutic interventions for individuals with substance use and co-occurring mental and physical health conditions. The NP/TNP D&A will function autonomously and collaboratively to provide safe, high quality advanced nursing care, within a multidisciplinary framework, contributing to service innovation, clinical leadership, education, and quality improvement initiatives. The NP/TNP D&A will primarily perform nursing assessment, management, treatment and clinical support to clients with substance use disorders. The NP/TNP D&A will contribute to the advancement of quality health care systems within the Local Health District to reduce avoidable hospital presentations, promote early discharge from hospital and maintain a high level of clinical care to patients . The core aspects of the role will be the delivery of safe, quality practice including the development of specialist plans of care that meet the needs of the client in collaboration with the medical team. The NP /TNP D&A role includes the assessment and management of clients using nursing knowledge and skills and may include but is not limited to initiation of diagnostic investigations, prescribing of medications and direct referral of patients to other health care professionals. The NP /TNP D&A will practice within the NP /TNP D&A ’s approved scope of practice (ScOP), competence, legislative and policy frameworks and the strategic direction of the service and CCLHD. In this role, you will provide advanced clinical nursing care and interventions within the authorised scope of practice and in accordance with the requirements of the NSW Nurses and Midwives Registration Board, Nursing and Midwifery Board of Australia (NMBA), and relevant organisational policies and procedures. You will develop, implement, and maintain an approved Scope of Practice (ScOP) aligned with NMBA standards and organisational Nurse Practitioner policies, clearly identifying the clinical context, role boundaries, and evaluation strategies for advanced practice, develop, monitor, and evaluate measurable Nurse Practitioner practice outcome indicators, and regularly review and refine own Scope of Practice to ensure safe, effective, evidence-based, and contemporary clinical care. You will provide patient-centred advanced nursing assessment, diagnosis, treatment planning, and management strategies in collaboration with medical officers, nursing staff, allied health professionals, and multidisciplinary teams to support optimal health outcomes, provide education and clinical advice to clients, carers, and families regarding health conditions, treatment options, medication management, harm minimisation strategies, and recovery-oriented care within the clinical environment. You role incluses to facilitate and support the involvement of families and carers in assessment, care planning, treatment, discharge planning, and ongoing management strategies, ensuring the provision of appropriate education, information, and support services. You will establish and maintain effective communication pathways and collaborative working relationships with services across the health district, including Mental Health Services, acute medical and surgical teams, community health, and primary care providers and you will develop strong professional networks and collaborative partnerships with General Practitioners (GPs), pharmacists, non-government organisations (NGOs), community agencies, and other relevant stakeholders to support continuity of care and coordinated service delivery for clients. You will have: Current registration as a Registered Nurse with AHPRA and current endorsement as a Nurse Practitioner or evidence of working towards endorsement. If not endorsed, the Transitional Nurse Practitioner will be appointed temporarily (up to 3 years) to work towards endorsement as a Nurse Practitioner. Demonstrated capacity to integrate the principles of clinical governance, quality improvement, evidence based practice, patient centred care and practice development into the delivery of advanced nursing practice. Demonstrated capacity to deliver advanced nursing practice in the field of Drug and Alcohol and independently assess, investigate and implement appropriate treatment or referral for patients within advanced nursing practice guidelines and other relevant NSW Health guidelines. Demonstrated experience as a clinical leader with commitment to ongoing development of safe clinical practice to an advanced level.
