The Palliative Care Social Worker will provide support for people with a life-limiting condition and their families within Murrumbidgee Local Health District acute care facilities. The Social Worker is an integral part of the MLHD Palliative Care Team, working alongside virtual patient liaison nurses to provide multidisciplinary social work input, enhance early palliative care services, and ease transition of care to the community.
