As a Community Support Worker at Astute Living Care, you will provide person-centred support to clients within community and home-based settings. You will assist individuals with daily activities, social participation, and wellbeing support while promoting independence, dignity, inclusion, and positive community engagement. Responsibilities and Tasks: Assist individuals in connecting with appropriate community, health, and support services to meet their personal and social needs. Encourage clients to develop daily living skills, increase independence, and remain actively involved within the community. Liaise with family members, service providers, and other stakeholders to ensure clients received suitable ongoing support. Provide information and practical assistance to clients when accessing available programs, services, and community resources. Observe and reported changes in client circumstances, wellbeing, and support requirements to relevant team members. Prepare and maintained client documentation, including support records, case updates, and incident information. Promote respectful, inclusive, and client-focused support practices in line with organisational values and individual preferences. Assist with the delivery of tailored support arrangements to help clients achieve personal goals and improve quality of life. Encourage participation in social, cultural, and recreational opportunities to strengthen community involvement and reduce isolation. Work alongside care staff and support teams to help maintain effective and consistent service delivery. Qualifications and Skills: Relevant qualification. • Minimum 1 year experience as a Community Support Worker. • Demonstrated understanding of person-centred care principles and community support practices. • Excellent communication skills, empathy, and ability to work with diverse client needs. • Ability to manage documentation, prioritise tasks, and work effectively within a collaborative team environment. Salary: The role offers a remuneration package ranging from $75,000 - $85,000 p.a plus superannuation for a 38-hour work week.
