With a booming, culturally rich and diverse community, and less than an hourâs drive from the CBD (still within the bounds of Myki and ride-sharing), Melbourneâs North is fast becoming the place to be.   Northern Health are seeking applications from motivated and suitably experienced Grade 3 Physiotherapists or Occupational Therapists to join the Older Persons Care Hub.  Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourneâs rapidly growing outer north.  Services are provided through our six main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.  About the Role: The Older Persons Care Hub (OPCH) aims to provide rapid short- term interventions to older people in their homes who are at high-risk of hospital presentations.   The Grade 3 Physiotherapist or Occupational Therapist is responsible for providing interdisciplinary assessment and intervention for older adults presenting with frailty and or falls issues.  This role is responsible for: The OPCH is a rapid response short term program for older adults living in the community who are at risk of presenting to hospital with frailty and or falls risks.  Services will be provided in the clientâs home and involve short term intervention, such as provision of aids to address or minimize falls risks.  The interdisciplinary team will provide relevant falls education and facilitate referrals to other services within the community. The Grade 3 Physiotherapist or Occupational Therapist will carry a caseload and will be recognized as a clinical expert in falls assessment and management.  The Grade 3 Physiotherapist or Occupational Therapist will be required to work Saturday, Sundays and public holidays. Job share will be considered. The hours of work would be 8.00am -4.30pm.  As Allied Health services and programs at Northern Health continue to develop and evolve to meet the increasing demand of servicing the Northern growth corridor the role will require staff to be flexible with their working hours and may include working after hours. The senior clinician maybe required to travel and work across the various Northern Health campuses, programs, and partner organisations.  Compliance with mandatory training as per NH Policy is a requirement of this role. Failure to complete this training within required timeframes may result in a suspension of duties until training is complete  To be successful in this role you will have: A minimum of seven years of relevant clinical experience Demonstrates expertise in falls and falls management Proven experience to work with complex clients and working understanding of community referral pathways Demonstrated highly developed interpersonal skills Demonstrated ability to promote evidence-based practice Possesses practical negotiation and conflict resolution skills  Next Steps . . .
