Mandatory requirements: Cert IV in Communities or approved equivalent or ability to undertake the Cert, Drivers Licence, Provide First Aid certificate Key responsibilities: Develop, implement and evaluate support plans to assist clients to improve and maintain life skills increasing positive behaviours and improving quality of life. Supporting clients to develop skills required for independent everyday living. Observe clients who are at risk to self and others, identify and provide relevant information surrounding risk concerns. Support each client to achieve the goals within their IDP. Implement treatment programs to address offending behaviours, and Implement habilitation programs. Support clients to develop personal routines and lifestyles to meet their individual needs, abilities and preferences. Contribute to the development, implementation and review of risk assessments, health plans, medication plans, individual development plans, limited community treatment, and transition plans. Prepare reports, maintain documentation, and undertake administrative tasks and data collection. Ensure the safety of clients, staff and visitors including observations, searches, duress response and participate in managing challenging and aggressive behaviour. It is vital to have the ability to be emotionally resilient in achieving objectives and coping effectively with challenges and incidents. Support clients to access the community. Adhere to legislation, policies, procedures and practices and work within the legislative framework. Adhere to decisions made by Operational Team Leaders and Senior Leadership Team. Identifying, developing and implementing continuous improvement initiatives to improve quality of support to clients. Support clients with medicines according to directions from a qualified medical practitioner. You may exercise powers by appointment under the Forensic Disability Act.
