What you'll do: Conduct assessments, reviews, coordinate care plans and nursing interventions tailored to individual customer needs. Support the utmost wellbeing of customers living in their own home Provide nursing interventions such as wound care, medication management, continence care, and management of chronic conditions Observe and evaluate customer wellbeing, taking timely and appropriate actions in response to clinical changes. Document nursing care accurately, thoroughly, and in a timely manner Participate in on-call after hours service Liaise with internal and external stakeholders including family members, Support Workers and Allied Health Professionals to support customers About You: You are a talented and passionate Nurse. You want to make the most of your expertise, do what you do best and thrive on the challenge of providing our customers with quality care.
