Your part in our Statewide Specialised Team Reporting to the Dementia Care Manager, Statewide Specialised Service, you will support BlueCare’s neighbourhoods in providing support, guidance and strategies to personal carers, in the delivery of high quality in-home dementia care. With our clients’ wellbeing always in mind, you will deliver direct in-home or telehealth support to people living with Dementia and their families by providing: Information and tools specific to individual circumstances. Assistance to access BlueCare support services and programs to maintain health and well-being. Carer Support. Techniques to cope with individual behavioural concerns. Advise meaningful activities and equipment from a comfort interventions library, to engage people living with Dementia. Under the guidance of the Dementia Consultant Care planning and assessment as per review schedule and change in clients’ needs. What makes a difference for us? Coupled with high level listening and problem solving skills, you will have the ability to develop an understanding of the client’s needs and providing quality advise in ensuring our client’s ability to self-manage and live more independently. You’ll present with: Relevant qualifications and/or demonstrated experience in the delivery of a community-based service or health related field especially in Dementia. Cert IV or higher level of qualification will be considered. Demonstrated experience of working with carers of people with dementia, people living with dementia and their families; and demonstrated ability to provide recommendations/suggestions to improve the lives of people living with dementia. Demonstrated high level of verbal, written and interpersonal communication Demonstrated ability to network with, consult and respond to the needs of a diverse group of stakeholders.
