We are seeking passionate and driven Home Navigator to support individuals to access safe, stable and sustainable housing. Your part in our UnitingCare team The Home Access for people with Disability program's objective is to ensure that people with disability have access to secure, suitable, and stable housing. By providing comprehensive support and capability building through a collaborative, multi-agency approach, the service aims to facilitate seamless transitions into appropriate housing solutions, enhancing the quality of life for people with disability. In this role, you’ll work alongside individuals, their families, and support networks to expolore housing options, develop personalised housing pathways, and connect them with the right supports to achiveve their goals. This is more than a frontline role – you’ll also contribute to strengthening the sector by sharing your expertise, mentoring others, and supporting capability- building across the disability, housing, and homelessness sectors. As a Home Navigator, you will: Support individuals to identify and secure crisis, transitional and long term-housing options. Develop tailored housing pathways aligned to individual needs, goals and circumstances. Work collaboratively with housing providers to ensure reasonable adjustments and sustainable tenancy outcomes. Provide early intervention and tenancy support to help prevent evictions Build strong relationships with clients, families, and support networks Contribute to capacity-building initiatives, including mentoring and knowledge-sharing with partner organisations Support professional development across the disability housing and homelessness sectors. What makes a difference for us? You’re someone who is passionate about improving housing outcomes and confident working across complex systems. A Bachelor’s Degree in Social Work, Human Services, Disability, or related field with at least 2 years’ housing sector experience. Current Working with Children Check (Blue Card). NDIS Worker screening check Current Drivers Licence.
