About the role: Our Townsville and Community Service are seeking Full-time/Part-time Clinical Nurses to provide comprehensive specialist nursing care, clinical expertise and leadership. The overall purpose of the Clinical Nurse, Community Aged and Disability (CAD) is to provide comprehensive specialist nursing care, clinical expertise and leadership. The role will support the service as a clinical and management resource in delivery of quality and best practice nursing care while leading, motivating and developing the Service’s staff as well as maintaining and building relationships with key stakeholders. The role may also be required to act in the role of Multidisciplinary Team Coordinator and/or be on call. The pay-rate for the role will range between $61.13-$63.29/hr depending on your experience. Have, maintain and apply advanced practice knowledge in clinical areas relevant to Community Nursing, e.g. wound care, catheter care, stoma care, insulin management. Manage clinical workload including care plans, referrals and provision of services Ensure communication with clients and carers is responsive to the situation From a base of clinical expertise, provide consulting and coaching to staff in relation to specific cases or aspects of clinical practice Make significant contribution to maintaining high standards of clinical practice within the Service / Program and undertake a client load under direction of Multidisciplinary Team Coordinator Implement a recognised best practice problem solving methodology when addressing individual actions which cause concern Contribute to review of organisational clinical practice by participation in appropriate clinical forums Assist with implementation of new activities, systems and other organisational changes. This may involve review of Service based operating systems and implementation of identified refinements/improvements Coordinate resources for effective operation of the nursing services Assist with clinical administration – client file reviews, program related documentation, audits, claims preparation and submissions etc. Role Requirements: Valid Aged Care Worker Screening Check: o Current National Police Check OR o Current NDIS Worker Screening Clearance. NB: NDIS Worker Screening Clearance is mandatory for Services/Facilities delivering services to NDIS clients. o Statutory Declaration if you have been a citizen or permanent resident of a country other than Australia after turning 16. Current Drivers Licence and own reliable vehicle Degree in Nursing Current CPR Certification Current AHPRA registration and evidence of recent practice Postgraduate qualification in Health or other relevant/specific area of clinical practice – desirable Ability and willingness to travel within the service region Anglicare strongly encourages you to stay up to date with the immunisations recommended for your role, as outlined in our Worker’s Immunisation Procedure About you: Specialised knowledge and expertise in the area of community nursing across the lifespan including nursing assessment, case management and evaluation Demonstrated leadership and management skills at a work group level and the drive and experience to lead a nursing team Excellent interpersonal/communication and analytical skills (written and oral), coupled with training, coaching or mentoring skills and experience Understanding of funding programs which may include HCP, CHSP, QCSS and Fee for Service Working knowledge of accreditation standards and proven ability to actively and positively participate and contribute to Continuous Quality Improvement activities and systems review and operation Able to demonstrate well established networks and knowledge of resources available in community care Why Join Anglicare?
