Our innovative service features are designed to help patients take those crucial first steps towards wellness, but also to support their ongoing wellbeing and promote a sustainable recovery. Our mission is to provide wrap-around and human-first mental health care that meets people where they are at in their recovery journey, at the right time, no matter what stage of healing they’re in. We are looking for several passionate and innovative Mental Health Social Workers to join our team who share our vision for a new kind of mental health care. Our approach. · An interdisciplinary care team consisting of psychiatrists, nurses and allied health professionals (e.g. psychologists, occupational therapists, social workers, physiotherapists and dieticians) with both profession-specific and shared care responsibilities. Unlike siloed multidisciplinary care, interdisciplinary care involves two or more clinicians using an integrated approach, working together to achieve common goals and provide care across different environments at individual, family and community levels. · Close-to-life mental health care that is embedded in facilities within the Murdoch Square mixed-use precinct, allowing patients to access mental health services whilst remaining close and connected to their families, support network and present in the community amongst a range of amenities. · An enactive model of mental health care, which acknowledges that patients actively shape their well-being through personal interactions with their environment. This dynamic approach allows us to move beyond the biopsychosocial model and honours the unique and human nature of our patients. It recognises that mental health is a continuous process influenced by nuanced interactions between biological, social, cultural, psychological and environmental factors. · Care coordination services undertaken by the most appropriate clinician in a patient’s care team. These services include case management and psychosocial support during admissions, transitions in care and post-discharge to address the holistic needs of our patients. It aims to improve mental health outcomes, support sustained community recovery and reduce hospital readmissions through comprehensive discharge planning, early intervention, stakeholder collaboration and connecting people to community-based services. · A graduated care pathway that offers diverse lower and higher intensity treatment and support options, including both inpatient admissions and an outpatient clinic with individual therapy and a range of group therapy sessions and programs. · Holistic risk streaming and management to match the right care to patients at the right time. Our streaming approach identifies individuals with more intensive support needs or complex care requirements – and facilitates proactive care and early intervention to prevent, reduce and support challenges or deteriorations during admissions and following discharge. · A diverse range of innovative and evidence-based group therapy options available through the Aegis Health Clinic and Aegis Health Day Clinic. · Practising community-integrated psychosocial care and use of community-based partnerships to enhance outcomes and continuity of care. Facilities. Aegis Health Clinic The Aegis Health Clinic is a 36-bed inpatient mental health unit situated within Aegis Health Private Hospital. It is an unauthorised facility that offers overnight admissions for patients requiring higher intensity treatment, support and care in a hospital environment. Our inpatient stays include access to variety of group therapy sessions that are individualised and may involve care coordination services for those with higher support needs. Our group program is designed to stabilise mental health, reduce distress, impart fundamental recovery knowledge, build skills and enhance daily functioning – to provide a strong foundation for long-term recovery after discharge. Aegis Health Day Clinic The Aegis Health Day Clinic is central to our innovative graduated care pathway and is situated in our private consulting suites. The Day Clinic offers outpatient services, including group programs, individual therapy and an ADHD assessment service for adults – all of which are aimed at fostering independence, self-efficacy, enhancing function and gaining skills whilst remaining connected to their daily life and support systems. Populations. Our target populations are individuals with mental health conditions or difficulties who require support on a voluntary basis, including those with comorbid alcohol and drug issues. Funding groups are private health members, youth and young adults (16-24 years), workers’ compensation, ICWA, veterans and first responders. Aegis Health Candidates. We value allied health professionals who bring expert knowledge, depth of experience and specialised skills in treating and managing mental health conditions. This enables us to form highly capable care teams tailored to the unique needs of our patients. Social Workers must hold relevant tertiary qualifications and a full membership and mental health accreditation with AASW. Ideally, we are seeking Mental Health Social Workers with at least three years of experience working in the mental health sector. Mental Health Social Workers who have worked in an inpatient setting and/or those who are training and/or experience in undertaking group-based or individual Cognitive Behavioural Therapy (CBT), trauma psychotherapies, interventions for neurodiverse populations, and/or Dialectical Behavioural Therapy (DBT) are highly desirable. 2. Commitment to integrated care. Ideal candidates are enthusiastic about and open to working together within an interdisciplinary team, including having shared responsibilities. We're searching for Social Workers who can collaborate across professions to align and provide comprehensive and holistic wrap-around care for our patients. 3. Innovative mindset.
