About the job OCCUPATIONAL THERAPIST – Dementia Enablement and Family Support Memory Nurture Pty Ltd is a mobile Therapy Service and is a Registered NDIS provider, specialising in the NDIS, Aged Care in the community as well as clients residing in Aged Care facilities and NDIS SIL [ Supported Independent Living ] accommodation. Part time experienced therapists required in : Perth North, Metro and South Are you passionate about enriching the quality of life and well being of those living with memory loss or dementia as well as providing support for families ? Memory Nurture is seeking experienced and compassionate Occupational Therapists to join our team of OTs, PTs, and Music Therapist. Therapy assistants and Support Workers encourage clients to achieve their goals under the supervision of an OT and /or PT. Minimum 3- 5 years of Occupational Therapy experience is required for this position A warm, caring nature and good communication skills are essential to be successful in this role. Experience with equipment and manual handling skills highly valued. Knowledge of dementia care, acquired Brain Injury, CVA. Parkinson's disease valued as well as an interest in Telehealth is also welcome. Specific dementia or aged care experience is preferred. Job Responsibilities Evaluating clients physical, cognitive, social, emotional status and abilities via clinical observations and standardised testing e.g. NDIS Functional Capacity assessments Providing clients with assistive devices where required (e.g. Hoists, wheelchairs etc ] where necessary. Evaluating clients functional status in their home, work and leisure environments Establishing treatment goals and planning treatment with the client and family. Implementing the plan of care through direct treatment and supervising Therapy Assistants/ Support workers. Coordinating treatment with other healthcare professionals Providing guidance and education to client, family, and caregivers Evaluating need for interdisciplinary referral Recording, maintaining and providing accurate, timely documentation of client treatment and progress.
