In this role, you will deliver advanced, patient-centred care, focusing on teaching patients and their carers to perform haemodialysis at home, for people living with end stage renal disease. In this role, you will: Education and Training: Training patients and carers on home haemodialysis (HHD) Fresenius 4008 machines, techniques, and troubleshooting. Support and Monitoring: Providing ongoing clinical support, often including telehealth or home visits, to manage vascular access (AVF/AVG/CVC) and assess treatment effectiveness. Advocating for patients, while supporting innovative models of care, to reduce avoidable hospital presentations. Patient Autonomy: Empowering patients to manage their own care, improving quality of life, and offering flexibility. Clinical Environment: This role is based in the hospital outpatient settings, with some travel to patient homes and other sites. Multi-disciplinary: Working and collaborating care for the patient and carers, alongside multi-disciplinary team members, for patient centred care outcomes. For a full list of role responsibilities, please review the Position Description About You Our ideal candidate will demonstrate: Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as a Registered Nurse Post graduate qualifications relevant to renal nursing, and at least 3 years fulltime equivalent clinical experience. Including proficiency in vascular access cannulation Demonstrated ability to coordinate complex care and support patients managing chronic conditions across hospital and community settings Strong clinical leadership skills and ability to mentor and support staff Excellent communication, collaboration and care coordination skills within multidisciplinary teams Commitment to evidence-based practice, quality improvement and ongoing professional development Current Class C driver’s licence and willingness to travel across the district if required Why join MLHD?
