Description The opportunity: We are looking for an experienced Care Coordination Team Lead to provide clinical leadership within an innovative, time-limited health initiative supporting people living with chronic and complex conditions. This is a fixed‑term role until 1 July 2027, commencing in June 2026. The first six months (setup and proof of concept) will be at 0.5 FTE, increasing to 0.8 FTE as the initiative progresses into delivery. This is a hands-on leadership role , ideal for a clinician who enjoys improving how care is coordinated across services while supporting clients to build confidence, capability and momentum through health coaching. In this role you will: Lead day-to-day care coordination, health coaching and navigation activities for clients with complex and chronic needs Provide support, guidance and coordination for a small team delivering client-centred care Partner with clients to develop goal-directed care plans and strengthen self-management and health literacy Apply behaviour change approaches (e.g., motivational interviewing, goal setting) to support sustainable outcomes Work collaboratively with internal and external stakeholders (e.g., primary care, allied health, community services) to support coordinated care Contribute to continuous improvement, helping refine workflows and ensure consistent, safe practice Maintain accurate documentation and support routine tracking of service activity and outcomes Skills And Experiences Our ideal candidate: Registered Nurse with AHPRA Bachelor of Nursing or equivalent qualification.
