DNA Healthcare is currently seeking a Clinical Case Manager to join a well-established home care team in Darwin. This is a great opportunity to make a meaningful impact by supporting older Australians and clients with complex care needs to live independently and safely at home, paying a salary package of up to $106K NFP Salary packaging. About the role You will be working within a respected organisation that has services across acute hospitals, residential aged care sites, and community care. In this role, you’ll manage a caseload of CHSP and Home Care Package clients, coordinating services, managing budgets, onboarding new clients, and ensuring care plans are delivered effectively to support client wellbeing and independence. This role suits someone who enjoys a busy, client focused environment and is passionate about quality care outcomes. Key responsibilities Ensure compliance with Aged Care Quality Standards. Coordinate and liaise with internal teams, external providers, and key stakeholders. Oversee care planning, budgeting, and ongoing client reviews. Support the smooth onboarding of new clients. Maintain accurate documentation and high-quality case management practices. About you 2 years’ experience working in a community setting as an RN. Strong experience supporting clients with complex care needs. Knowledge of the My Aged Care portal (highly regarded). Relevant tertiary qualification in health/community services (desirable). Strong communication, time management, and stakeholder engagement skills. Current driver’s licence and reliable vehicle. Passion for person centred care and positive client outcomes.
