About you Hold a Cert III in Individual Support or Community Services At least 12 months of proven experience in care coordination. You have a full driver's licence and access to a reliable vehicle with comprehensive insurance, and you have the right to work in Australia. You are ready to start as soon as possible, bringing role-ready skills, and you are comfortable with occasional on-call shifts. Strong instinct for matching carers to clients and rostering effectively, along with solid skills in documentation, budget management and conducting client visits.
