Integrated Family Services Practitioner 0.8 EFT (4 Days per Week) | Wyndham Vale 3-Month Locum Contract → Potential Permanent Role We are currently assisting with recruitment for an Integrated Family Services Practitioner to support families through case management, goal planning, and safety planning within an Integrated Family Services program. This role is based in Wyndham Vale and is initially a 3-month locum contract, with the intention of transitioning to a permanent position for the right candidate. About the Role In this role, you will work directly with families to improve safety, wellbeing, and family functioning through structured case management and support. You will develop goals, action plans, and safety plans with families and provide direct support to individuals and families, and facilitate group work where required. Key Responsibilities Provide case management support to families Develop goals, action plans, and safety plans with families Work directly with individuals and families through outreach or office-based support Facilitate group work when required Liaise with partner services and make appropriate referrals Maintain case notes and documentation About You To be successful in this role, you will have: Experience working in Integrated Family Services, Child & Family Services, or Family Violence Case management experience Ability to complete assessments, safety planning, and goal setting with families Relevant qualification in Social Work, Community Services, Psychology or related field Understanding of the Child Youth and Families framework (desirable) Role Details 0.8 EFT (4 days per week) Location: Wyndham Vale Initial 3-month locum contract Strong potential to become permanent SCHADS Level 5.1 - 5.2 $63 - $66 per hour (depending on experience) This is a great opportunity for someone looking for a part-time role with the potential to move into a permanent position.
