You’ll empower individuals to live more independently, safely, and meaningfully by: Actively listening to and addressing the needs and concerns of at home aged care community clients, ensuring their preferences are respected and incorporated into care plans. Ensure clear, compassionate, and professional communication with residents, families, to ensure optimal customer experience and satisfaction. Conducting thorough and competent holistic assessment, care planning, direct nursing care and review cares using a biopsychosocial model to optimize customers independence, wellbeing and provide comfort at the end of life. Reflect on practice and demonstrate competent decision making and problem solving, ensuring the delivery of high-quality care. Provide workplace coaching, guidance and supervision to Personnel Carers, Assistant in Nursing and Enrolled Nurses, and act as a role model for practice and professional conduct. Continue to develop knowledge and skills in specialty care (including but not limited to aged care, young adults, people with significant disability, palliative care, pain, wound management, infection control). Be able to prioritise your workload safely and efficiently with focus on customers. Work with CN/CNCs/AMs and the Manager to investigate and process any complaints in accordance with organisation policy. Achieving outcome standards and service delivery practice per Aged Care Standards, NMQIP, Government regulations, funding bodies and other relevant accreditation requirements. What We’re Looking For Current registration with the Australian Health Practitioner Regulation Agency. Demonstrated proficiency in clinical nursing practice with an understanding of contemporary nursing theory and care documentation models. Generalist registered nursing experience with knowledge of aged care highly desirable. Ability to rapidly acquire a sound understanding of appropriate aged care legislated standards and incorporate into daily practice. Ability to lead a care team to provide coordinated care using a biopsychosocial model of care. Demonstrated high level of written and verbal communication skills, including team building and day to day management. Commitment to the professional development of self and others, including the ability to develop and foster an understanding, commitment to the principles of continuous improvement and person-centred care.
