As a Specialist Local Area Coordinator (LAC), you will provide tailored support to participants with high support needs, multiple disabilities or significant barriers to participation. This role is place based, community facing and focused on delivering intensive, high quality services that promote inclusion and independence. Why Feros Care? What you'll do: Deliver tailored support to participants with complex needs, ensuring their NDIS journey is accessible, empowering and aligned with their goals. Coordinate intensive planning and navigation across scheme access, plan development, implementation, reviews and check ins. Build strong relationships with participants, families, carers and local support networks to foster independence and sustainable community connections. Facilitate access to specialist services, clinical supports and mainstream resources that meet individual needs. Manage a specialised caseload and ensure that your service delivery meets NDIA benchmarks, approved activity hours and Feros Care's internal performance expectations. Engage with external stakeholders, such as clinical providers, community organisations and service networks to support complex participant scenarios. Maintain accurate records and reporting using NDIS and Feros Care systems. Continuous improvement practices through participating in audits, reviews as well as thedevelopment of best practice approaches. Participate in team planning, professional development and initiatives that advocate for theparticipant voice. What you'll bring: Extensive experience supporting people with complex disability, high support needs and/or multiple barriers. Strong understanding of the NDIS participant pathway and planning processes. Case management skills including risk assessment, goal planning, and stakeholder coordination. Excellent communication, interpersonal and problem-solving skills. Proficiency in data entry, reporting and case management systems. Ability to work independently in community-based settings. A commitment to person centred practice, inclusion, and continuous improvement. Qualifications & Checks: Tertiary qualification in community services, disability, social work, or a related field. Additional qualifications or experience in complex disability, case management or high support needs. Access to reliable transport to travel locally. NDIS Workers Screening Check & Working with Children's Check (or willingness to obtain). Undergo a NDIA Security Clearance. Undergo a Nationally Coordinated Criminal History Check. Valid Australian Working Rights. Lived experience of disability is highly valued.
