These include: Providing nursing care with the support from an Experienced Registered Nurse Collaborating with community supports, referrers, GPs, families and allied health Contributing to the assessment, prevention and early detection of any health conditions Contributing to the formulation of care plans, reviews and case conferencing Joining the team, you’ll work across various suburbs in Melbourne’s Western Region in a full-time capacity, following a five-day roster that includes evenings and weekends.
