The objectives of the program are to establish a national Care Finder network, support and promote continuous improvement of the Care Finder program and support improved integration between the health, aged care and other systems at the local level. About the Role The Case Worker is responsible for providing assertive case management, education and support to participants in order for the program to meet its objectives. The CareFinder Caseworker is responsible for engaging with vulnerable older community members who are eligible for aged care services and who require support in accessing these services. Caseworkers have an in depth understanding of the aged care system and the various supports available. Caseworkers are able to support eligible participants to understand, access and engage with the relevant services and programs which support their individual health and wellbeing. What we are looking for Relevant qualification in certificate IV in Aged Care or similar including certificate iv in Community Services Strong interpersonal and communication skills including the ability to engage with a wide range of people and professionals. Experience of working with interpreters. Ability to engage with vulnerable populations such as isolated individuals experiencing trauma, complex health issues, cognitive impairment, homelessness or unstable housing.
