Our Clinical Care Coordination services provide intensive, wrap-around supports for people experiencing severe and persistent mental health challenges, bringing together clinical care, psychosocial support, and access to essential community services. By integrating clinical care, psychosocial support, and access to essential community services, we aim to provide holistic, person-centred care that promotes recovery and wellbeing. We’re seeking a compassionate and skilled Clinical Care Coordinator in Rockhampton to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region. Working closely with a multidisciplinary team, you’ll support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, collaborative practice.
