About the role This is a permanent, full-time position located in Wagga Wagga. The Care Navigator, Medicare Mental Health Centre Wagga Wagga works collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care. Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems. Demonstrated understanding of supports and services available in the local and regional communities. Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
