Job Description Support at Home program Our team provides a range of services to ensure effective and safe care to Older Adults. The Team encompasses Case Management, Service Coordination, Administration and Direct Care Supports. We strive to optimise the health and quality of life of our clients through providing coordinated, person-centred, supportive care. The role Located across the Eastern, Outer Eastern and Yarra Valley regions. Office based in Ferntree Gully. Hybrid work model (home visits office WFH, where approved). Part-Time (0.8FTE) ongoing position – days negotiable. $ 44.78 - 48.99 per hour Not-for-Profit Salary Packaging to maximise your take home pay As a Care Partner you will be providing Older Adults, their families and carers with access to responsive care and case management, ensuring the customers receive services and support tailored to their needs and requirements. On a typical day you will liaise with older persons, either on phone or in their home, to follow up on their care requirements. Your conversations will revolve around their goals to achieve happy, fulfilled lives whilst remaining independent in their own homes. Responsibilities: Utilising your case management skills to undertake initial and ongoing comprehensive assessments and care planning to support clients at home. Managing the delivery of services to support the needs identified (psychosocial, medical, physical and vulnerabilities) within the customer’s individualised budget. Drawing upon and building on relationships with colleagues, services networks and a range of community agencies to support the program and customers. Who we are looking for If you’re an experienced Case Manager / Care Partner, qualified in Community Services, Health, Aged Care or related discipline and enjoy assisting older adults with their day to day needs through accessing quality care providers, you might be exactly the person we’re looking for. To succeed in this role you will: Australian Drivers Licence 2 years’ experience in Case management in home and community based settings Qualifications in Aged Care, Health, Community Services or related discipline (highly desirable) Your experience in building professional relationships with customers, their supporters and external service networks demonstrating an understanding of My Aged Care will ensure your success in this role. Holding similar values and principles associated with the provision of care for older adults and Each service principles will be invaluable. Your understanding and knowledge of the Aged Care Quality Standards, Community Home Care / Support at Home will support you to deliver a high level of service to older people.
