As a Referral Pathway Coordinator, you will work with our Next Steps team to promote, coordinate, and refine referral pathways and intake procedures into suicide prevention services across South-Eastern NSW and South-Western Sydney regions. Collaborate with teams and referring agencies to refine referral pathways and remove barriers. Screen referrals, conduct initial assessments, and make first contact calls. Manage a caseload of consumers to ensure ongoing support. Skills & Experience Qualification in Mental health (Minimum diploma level) or peer work qualifications (or commencement or a willingness to work towards) Demonstrated skills and experience working in a direct service provision role in a suicide prevention or mental health service Demonstrated understanding of the operations of the public mental health services and of the legislative frameworks relevant to mental health consumers, families and carers Experience in the development, refinement and implementation of processes and procedures with good attention to detail About Grand Pacific Health What we care about most is supporting healthier, happier, more connected communities across NSW and the ACT.
