Conduct intake and risk assessments and escalate serious risk when required. Coordinate health, mental health and psychosocial supports across agencies and community services. Provide expert clinical advice, prepare case summaries and support colleagues with challenging presentations. What you’ll bring Degree in Nursing, Psychology, OT or Social Work (mandatory). Experience with complex client needs, mental health, disability or trauma‑informed practice. Excellent communication, documentation and stakeholder engagement skills. Strong judgement, resilience and ability to manage risk in dynamic situations The CSP provides voluntary, time‑limited case management for vulnerable veterans—using a clinical lens and trauma‑informed approach to build independence, streamline communication and connect clients to services that support wellbeing.
