About your role You will play a key part in coordinating, managing, and promoting best-practice clinical care for clients living with chronic disease, with a particular focus on renal health. You will support early identification and management of chronic kidney disease, contribute to renal care planning and monitoring, provide client education on prevention and treatment, and work collaboratively with multidisciplinary teams and external providers to ensure culturally appropriate, client-centred care. You will also contribute to safe and efficient resource utilisation and the development and implementation of chronic disease programs within the community. About You You will demonstrate strong clinical skills, sound renal knowledge, and a commitment to improving long-term health outcomes in Aboriginal communities.
