We are seeking a compassionate, motivated and highly organised Social Support Coordinator to join our District Services team. This rewarding role is responsible for coordinating and delivering high‑quality social support programs that promote wellbeing, independence and meaningful engagement for older adults and community members. About the Role As the Social Support Coordinator, you will lead the planning, delivery and evaluation of the Support at Home – Group Social Support Program . You will work closely with participants, families, volunteers and health professionals to design inclusive, person‑centred programs that align with participant goals and foster social connection and reablement. Your responsibilities will include supporting volunteers, completing initial assessments and Goal‑Directed Care Plans, monitoring participant wellbeing, maintaining accurate records, coordinating communication, and ensuring programs are responsive, safe and compliant with aged‑care requirements. About You You will be a caring, proactive and community‑focused professional who demonstrates: Excellent interpersonal and communication skills Strong client management skills and the ability to work both independently and within a team A patient, compassionate approach and commitment to person‑centred care Understanding of Aged Care Accreditation requirements and the Aged Care Act Competence in computer systems and documentation practices A current First Aid Certificate, Police Check, and Victorian Driver’s Licence Desirable qualifications include Certificate III in Allied Health Assistance, Individual Support, or Certificate IV in Lifestyle & Leisure (or progress toward).
