About This Role Your new role: As a Registered Nurse within the Aged Care Assessment Organisation – Brisbane South, you will play a key role in supporting older people to access the most appropriate aged care services through comprehensive, person-centred assessments and collaborative care planning. Undertake comprehensive assessments of older people with complex care needs in hospital, home and community settings. Work collaboratively within a multidisciplinary team to support access to Commonwealth-subsidised aged care services, including residential care and home care packages. Deliver safe, compassionate and person-centred care through effective assessment, planning, implementation and evaluation of care. About You You are a compassionate and skilled Registered Nurse who is committed to improving health outcomes for older people and delivering high-quality, person-centred care. Registered Nurse with current registration with the Nursing and Midwifery Board of Australia (NMBA) and demonstrated experience in aged care, community or geriatric nursing. Strong clinical assessment and critical thinking skills with the ability to support complex care planning and decision making. Excellent communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary healthcare team.
