The Direct Care Attendant (Residential) assists residents in Residential Care Homes by supporting daily living activities and promoting independence. Working under the guidance of the Nurse Unit Manager, you will collaborate with a multi-disciplinary team to provide safe, person-centred care and contribute to a positive and supportive environment. The Direct Care Attendant (Community) provides support for people who are aged, frail, or living with a disability, helping them remain safely at home. You will work closely with the coordinator and community health professionals to provide practical assistance, social engagement, and diversional support while promoting independence and well-being. The Lifestyle Coordinator plans, coordinates, and evaluates programs that enhance independence, engagement, and well-being for participants. Using a participant-directed care approach, you will deliver individual and group activities, support volunteers, and collaborate with a multi-disciplinary team to provide meaningful, person-centred social support. The Lifestyle and Activity Assistant supports residents to participate in social and diversional activities that promote independence and engagement. You will assist with group and individual programs, provide emotional support, and contribute to a positive, safe, and inclusive environment under the guidance of the Lifestyle Coordinator.
