About the role The Care Partner is an integral part of the Home Care team, critical to the delivery of quality care and services that meet the needs of the individual client. Responsibilities will include but are not limited to - Working closely with the client to facilitate the provision of care arrangements that are responsive to the individual's wishes and needs, and promote independence and wellbeing Overseeing the implementation of care and services as agreed with the client Monitoring & responding to the changing needs of the client Managing client Support at Home accounts, both short-term and ongoing budgets, ensuring appropriate use of funds to achieve desired outcomes Supporting clients to make informed decisions, including respecting their right to take risks Conducting in-home assessments & reviews Collaborating & communicating with clients, families, internal & external stakeholders Skills and experience Previous experience in Aged Care, including an understanding of the Support at Home\Home Care Package program Tertiary qualifications in nursing, allied health, social work, care management and/or related experience Strong organisational skills, with the ability to prioritise multiple tasks, meet deadlines & reporting requirements Sound IT skills Be self-motivated and able to work autonomously Strong attention to detail Ability to maintain confidentiality How to apply If this role sounds suited to you, please apply now with a covering letter and current resume.
