At Better Health Network, we are looking for a passionate Social Worker or Nurse to join our Innovative Health Services for Homeless Youth (IHSHY) outreach program, supporting young people aged 12–25 years who are experiencing, or at risk of, homelessness. This is a meaningful outreach role where you will meet young people where they are in youth services, housing settings, schools, and community spaces, providing holistic health support, advocacy, and care coordination to help them access the services they need to stabilise, recover, and build independence. About The Role This outreach-based role works directly with vulnerable young people across a variety of community settings including schools, youth services, housing services, and healthcare environments. You will provide holistic support including health assessments, care coordination, counselling, advocacy, and health education while working collaboratively with youth services and community partners. Your work will help young people improve their physical, emotional and social wellbeing, while connecting them with the services they need to thrive. Key Responsibilities Deliver assertive outreach support to young people in community settings including homes, housing services, schools and youth agencies Conduct holistic health assessments, care planning and health education Provide generalist counselling, advocacy and case coordination Identify and address barriers to accessing health and social services Collaborate with internal teams and external youth, health and housing services Contribute to youth health promotion programs and initiatives Manage a case load and contribute to program outcomes and service targets Participate in supervision, team meetings, and ongoing professional development About You You are a compassionate and resilient professional who is committed to working alongside vulnerable young people in a respectful, strengths-based way. Qualifications You will bring: Tertiary qualification in Nursing or Social Work Current AHPRA registration (for nursing applicants) Experience Knowledge of adolescent health and development Experience supporting young people with complex needs, including those experiencing homelessness Skills in risk assessment, harm minimisation and crisis intervention Experience working with youth, homelessness or community health services Skills Strong communication and relationship-building skills Ability to work collaboratively across multidisciplinary teams Excellent organisation and case management capability Cultural awareness and commitment to working with Aboriginal and Torres Strait Islander and CALD communities Experience developing or delivering youth health promotion programs will be highly regarded.
