Collaborate with schools, families, and community organisations to create meaningful pathways for students, ensuring they remain engaged in education, training, or employment. About The Role We are seeking a dedicated Transition Pathways Officer to support schools in developing strategies and partnerships that ensure students at risk of disengaging from education successfully transition into further education, training, or employment opportunities. Reporting to a management committee of school principals, you will play a key role in shaping the future of young people in the community. Key Responsibilities Include Supporting schools in developing policies and practices for students exiting school before completing Year 12. Assisting schools in creating effective retention and transition strategies for at-risk students. Providing case management and support to young people and their families to facilitate successful transitions. Building partnerships with employment services, community agencies, training organisations, and employers to create opportunities for students. Maintaining a reporting framework to track the progress of at-risk students and provide updates to school principals and regional personnel. This role is ideal for someone passionate about supporting young people, fostering community partnerships, and creating pathways for success in education, training, and employment. About You We are looking for a motivated and compassionate individual who can work collaboratively with schools, families, and community stakeholders to achieve positive outcomes for students. The Following Traits Would Be Beneficial Experience in case management or working with at-risk youth. Strong communication and interpersonal skills to build effective networks and partnerships. Knowledge of education, training, and employment pathways for young people. Ability to develop and implement strategies to support student retention and transition.
