This could be the role you have been waiting for Your role As an Allied Health Professional, you will be looking after the most important people in our organisation – our clients. This is a great opportunity Your tasks will include: Assess clients emotional, psychological, development and physical capabilities using clinical observation Plan and recommend services to support clients with vocational, recreational, remedial and social activities on an individual basis Liaise with our other healthcare professionals in overall case management of clients Assist clients in community linking, referrals and information provisions. Provide counselling and support Support with future planning, advocacy and documentation to support future planning. Aged care navigation and support for entry to residential care setting including information regarding cost of residential aged care.
