Task & responsibilities Assess clients’ needs, abilities, and goals in collaboration with clients, families, and other professionals. Develop and implement individualised support and care plans based on client needs and NDIS goals. Provide emotional, social, and practical support to clients in their homes and community environments. Assist clients to develop daily living and social skills, promoting independence and participation in community activities. Support clients with accessing health care, education, employment, transport, and social networks. Liaise with other service providers, community organisations, and healthcare professionals to coordinate care and support services. Monitor client progress and wellbeing, maintaining accurate case notes and reports in accordance with organisational and regulatory requirements. Advocate for clients’ rights, inclusion, and access to services and opportunities. Maintain a high standard of professional and ethical practice consistent with A Cara Community Care’s values and relevant legislative frameworks. Qualification & experience Minimum AQF Certificate IV in Community Services, Disability, or equivalent qualification (or higher). Demonstrated experience providing support or case management in the disability, aged care, or community services sector. Strong understanding of NDIS frameworks, person-centred approaches, and community engagement principles. Excellent interpersonal and communication skills with a compassionate and respectful approach. Ability to work independently and as part of a multidisciplinary team. Sound written and computer skills for record keeping and client reporting. Current Australian driver’s licence and reliable vehicle. Current NDIS Worker Screening Check, Working with Vulnerable People (WWVP) or Working with Children Check, and First Aid Certificate (or ability to obtain).
